Independent investigations into incidents of serious self-harm and serious assaults in prison custody

The Secretary of State for Justice commissions independent investigations into a small number of very serious incidents that occur in prisons, in accordance with Article 2 of the European Convention on Human Rights. In order to ensure an appropriate level of public scrutiny, the reports of these investigations and the plans setting out the action to be taken by HM Prison and Probation Service (and relevant partner organisations) in response to the recommendations are published here.
