PPO report on learning from self inflicted deaths 2007-2009

The Prisons and Probation Ombudsman (PPO) has published a new report entitled 'Learning from PPO Investigations: Self-Inflicted Deaths in Prison Custody 2007-2009. The report provides an overview of self-inflicted deaths and draws upon information contained within over 200 investigative reports.
